How does COBRA administration work?

Activation

1. Forma receives an elections notice from the COBRA provider.
2. Once terminated, the employee’s work email address will be replaced with their personal email address in Forma.
3. Submitting claims
   1. COBRA FSA and LPFSA - the member can access their Forma account using their personal email address.
   2. COBRA HRA - the member should reach out to [e-mail] to submit claims.

Termination

1. Forma receives a COBRA termination notice from the COBRA provider.
2. Forma terminates user account.
