If you offer pre-tax benefits through Forma, you’ll see a collection of pre-tax graphs on the dashboard page of Forma Admin.

Filtering data
You can filter employee data by quarter, month-to-date, year-to-date, or select custom and enter the start and end dates you’d like to see.
Employees enrolled in pre-tax benefits
The line graph displays the month along the x-axis and the percentage of enrolled employees along the y-axis.
The Employees enrolled in pre-tax benefits graph allows you to track what percentage of pre-tax eligible employees are enrolled in at least one pre-tax benefit. This data can be used to track pre-tax benefits enrollment rate.
The percentage value is calculated based on the following:
- Number of employees with active pre-tax accounts divided by the number of pre-tax eligible US employees
- Number of employees with active pre-tax accounts = employees whose account end date is after the end of the month
- Number of active pre-tax eligible employees = number of US employees who are active and eligible for pre-tax benefits whose effective date is before the end of the month
Enrolled employees who have spent pre-tax money
The line graph displays the month along the x-axis and the percentage of active enrollees along the y-axis.
The Enrolled employees who have spent pre-tax money graph allows you to see what percentage of active pre-tax enrollees have spent pre-tax money. This data can be used to track benefit utilization.
The percentage value is calculated based on the following:
- Number of employees who have spent funds divided by the number of employees with active pre-tax accounts.
- Number of employees who have spent pre-tax funds = the number of employees who have spent pre-tax funds within the month
- Number of employees with active pre-tax accounts (same value as the numerator in the employees enrolled in pre-tax benefits chart) = the number of employees who have a pre-tax account whose account end date is after the end date in the filter
Accounts
Filtering data
You can filter the account graph by account type. Forma does not currently support an all accounts view or HSA view.
Please note that the account filter does not apply to the employee charts.
Total number of pre-tax accounts by status
The bar graph displays the month and year along the x-axis and the number of accounts along the y-axis. There can be up to three bars on this graph, representing active, temporarily inactive, and terminated accounts.
The Total number of pre-tax accounts by status graph allows you to track how many total accounts, per account type, there are per month grouped by status (active, temporarily inactive, or terminated). This data can be used to see the number of accounts per plan over time.
Note: Accounts where the account end date is past but the run-out period is still upcoming will display as temporarily inactive.
The totals are based on account start date before the end of the month and account status as of the end of the month.
Example:
Let’s say you want to see the account status for an employee’s FSA between Sept 30 and Nov 30.
An employee has an FSA start date of Aug 1 and an end date of Oct 15. The FSA has a 30-day post-termination run-out period which ends on Nov 14.
- In the September graph, the FSA will be counted as one active account because the account start date is before Sept 30.
- In the October graph, the FSA will be counted as one temporarily inactive account because as of Oct 31, the account falls within the run-out period.
- In the November graph, the FSA will be counted as one terminated account because as of Nov 30, the account status is terminated because the run-out period has ended.
