The Employees section provides all details related to an employee and their benefits including benefits, demographic profile, and activity.
Benefits
On this page you can review the following details:
- Current balance of active employer-sponsored accounts.
- Current balance of FSA, LPFSA, DCFSA, and Commuter benefits. Note: does not display the balance HSA balances
- Employee (Annual) Election for FSA, LPFSA, DCFSA, and HSA
- Employer (Annual) Election for FSA, LPFSA, DCFSA, and HSA
- Employee Pay Period Election for Commuter Transit and Parking
- Employer Pay Period Election for Commuter Transit and Parking
- Account coverage dates
- Employee (payroll) Contributions YTD
- Employer (payroll) Contributions YTD
- Total Contributions YTD
Click the pencil icon in the top right corner to temporarily deactivate an account or terminate it by setting an end date. At the bottom, you'll find inactive accounts the employee was previously eligible for but is no longer using.
Profile
On this page, you can review an employee's profile details. The information shown is sourced from file data provided by you or your vendor to Forma.
Please note that information can be edited on this page but will be overwritten by demographic data. Please ensure any changes made in Forma Admin are reflected on subsequent files.
Activities
On this page you can review activity details for a given employee, including:
- Claims
- Card transactions
- Store orders
- Store subscriptions
- Profile updates
- Pre-tax contributions
Claims
Claims are separated by employer-sponsored and pre-tax. The list of claims are ordered by date submitted, and can be filtered by status.
To comply with the Health Insurance Portability and Accountability Act (HIPAA), some information associated with FSA, LPFSA, or HRA claims including receipts, notes, categories, and merchant details are hidden from administrator view. Forma will retain the data for 7 years.

Card transactions
View Forma Card transaction details for a given employee, separated by employer-sponsored and pre-tax. Transactions are ordered by transaction date, and can be filtered by status.

Store orders
On this page you can review Forma Store order details for a given employee. They’re ordered by transaction date and can be filtered by status.
Store Order statuses:
- Confirmed
- Pending
- Rejected
- Recurring
- Completed
- Approved
Store subscriptions
On this page you can review Forma Store subscription details for a given employee.
Store subscriptions statuses:
- Active: The subscription has been activated by the vendor and is in good standing.
- Paused: The subscription is paused (can be initiated by the employee or the vendor).
- Pending: The employee submitted a request to sign up for the subscription and Forma is currently working with the vendor to activate their account/membership.
- Pending cancelled: The employee submitted a cancellation request and Forma is currently working with the vendor to cancel the membership.
- Cancelled: The subscription was successfully cancelled with the vendor.
Profile Updates
On this page you can review profile updates for a given employee. Profile updates can be filtered by time range and field.
Pre-tax Contributions
On this page you can review pre-tax contributions, including payroll and rollover, for a given employee. Pre-tax contributions can be filtered by time range and account type.
