Account type does not exist
Error Code: CT013
Error Message: Funding Date is invalid
Ensure the employee has an active account that matches the ‘Account Type’ sent in the contributions file. You can double check what active accounts an employee has by logging into Forma Admin > Employees > Benefits. Be sure to create any pre-tax account using the elections file first before sending contributions.

The noticeable difference between the elections file and the contributions file is that, in addition to the ‘Account Type’ column, you also provide the ‘Plan Year’ for FSA, DCFSA, LPFSA in the elections file.
During contributions file processing, Forma uses ‘Funding Date’ to see if it falls within the ‘Plan Year’ to determine what account the contributions should be deposited to.
For example, say your plan year follows a fiscal calendar; the 2025 plan year goes from July 1st, 2025 through June 30th, 2026 and your 2026 plan year goes from July 1st, 2026 through June 30th, 2027. An employee may have the same account type over two plan years, a 2025 FSA and 2026 FSA. Contributions can only be made for the current plan year. Any rows with ‘Funding Date’ between 2025-07-01 to 2026-06-30 will be considered a 2025 plan contribution, whereas ‘Funding Date’ between 2026-07-01 to 2027-06-30 will be considered a 2026 plan contribution.
- John elected a 2026 FSA. He did not have a 2025 FSA
- The contributions file has a row for John with ‘Account Type’=FSA and ‘Funding Date’= 2026-06-30
- This row will be errored out as CT013 because John does not have 2025 FSA
- The contributions file has a row for John with ‘Account Type’=FSA and ‘Funding Date’= 2026-07-01
- This row will be processed into John’s 2026 FSA
To troubleshoot this error, first confirm where the contribution should be made. If the employee has an active pre-tax account that you don’t see in Forma, create the account using the elections file then reload the contributions file.
